Application for Personnel Dosimeter

The request for the badge must be initiated by the Authorized user. Please fill out and submit the Application for Radiation Dosimeter form:

• Campus Dosimeter Application for campus lab workers.
• Hospital Dosimeter Application for hospital and clinics workers.

If you have previously had a dosimeter or a monitoring program, please also fill out a request for previous dose history with your dosimeter application. We need this to retrieve records from other institutions and to keep your dose history complete.

Report a Lost Dosimeter

Fill out the Dose Estimate for Lost Radiation Dosimeter form.

Pregnancy Surveillance Program

This program is voluntary. Please email the completed form to the pregnancy surveillance program coordinator. Contact: radiationsafety@fpm.wisc.edu.

View your Dose History

• Link the Landauer website to view dose history
  • Fill in User ID, UWMDose, and password, tCL4a8BUt6, as a Returning User. 
  • Do not click on "Email my Password"
  • Do not use "Create Login"
  • Use your badge account and serial numbers (located on the back of your badge) to access your dose report. Do not enter the leading zero on the account number.
